Active, Affiliate, Lifetime, and Student Membership Information

HLA Birmingham Chapter     Membership Application

Active Member: Active membership may be held by an individual who is directly employed in management or administrative support services by an entity formally organized to provide or facilitate the provision of healthcare services. An individual employed by a management organization, hospital/hospital system, practice management firm or other business entity providing healthcare services. This includes consultants who are responsible for operations of one or more practices on an ongoing basis. Healthcare providers/clinicians who hold an active license in the state are also considered active members. Each active member shall be entitled to one vote to elect officers and Directors of the Corporation. Active members shall not have any other voting rights relating to any other matters whatsoever. Active members are eligible to hold office and to serve as a director. Active memberships are non-transferable. Active Members: $150/Year

Affiliate Member: Affiliate membership may be held (i) by a non-medical organization which provides products or services directly to health care delivery organizations, or (ii) by an individual who works for a non-medical organization which provides products or services directly to health care delivery organizations. Examples of non-medical organizations include accounting, legal, consulting, data processing, supplies and equipment and insurance organizations. An affiliate member shall have no voting privileges and is not eligible to hold office or serve as a director. In order to keep a balance between the Active and Affiliate Membership, the Board must approve all Affiliate members and at times affiliate memberships may only be approved for the following year Affiliate Members: $350/Year ($350/Year For Additional Memberships)

Student Member: Student membership may be held by an individual who is pursuing a healthcare or business-related degree at an accredited institution of higher learning and does not qualify for any other membership category. A student member shall have no voting privileges and is not eligible to hold office or serve as a director.  A student member may pay $10 for each meeting that they wish to attend.

Lifetime Member: Lifetime membership may be granted to a former active member who has been a member of the Corporation for at least 15 years and/or has made significant contributions to the Corporation. A life member shall have no voting privileges and is not eligible to hold office or serve as a director. Annual membership dues will be waived.
